1. Write down your story. Tell us how your new power chair or scooter has made a real difference in your life.
   
   
2. Print-out and sign the permission forms below. You’ll need to send these in for us to share your story with others who might benefit from hearing how your new power chair or scooter has made a difference in your life.

     The Assignment of Copyright Interest and Authorization for Use and Disclosure of Personal Information for Marketing Purposes forms simply give us permission to use your words and photos in promotional or advertising materials. Please sign and mail these back to us.

     The Notice of Privacy Practices form informs you about how The SCOOTER Store will use any health-related information you provide in your story in promotional or advertising materials. We are required by law to provide you with this form.
